Navigation Consent

Looking for help with navigation consent. I attended the mid-year conference which was beyond helpful. Currently I am the only oncology nurse navigator at a community hospital. Our physicians are independent, so they are not on site, they all have their own private practices then refer patients to the cancer center for treatment. From what I understood from the conference is that anyone can consent the patient for navigation services correct? It doesn't have the be the physician? Does anyone have an example of a consent they use? We are a brand-new navigation program, and I have zero experience establishing one.

I am a new nurse navigator! I just joined here and have been looking through things and saw your post. Our facility is just beginning to start the billing for navigation services, so they are working on a consent model to use. When that gets finalized, I would be happy to share the information with you.
