Mission: Through education, networking, resource and knowledge sharing we will enhance support and guidance across the continuum of care for people in our region who face a cancer diagnosis. Collaboratively we will address barriers to care and support care transitions across facilities by improving navigator connections.
Vision: We will be the driving force to educate and collaborate with other oncology nurses and patient navigators in our region to reduce barriers to achieving equitable cancer care.
Objectives:
- Establish a directory of Oncology Nurse and Patient Navigators in the region to increase connections between teams and facilities
- Support professional development by inviting expert speakers to our meetings
- Share workflows and tools across facilities to support the growth and efficiency of our teams
- Share community resources to increase supportive connections for patients and their caregivers
- Empower each other through knowledge sharing
Membership:
- No membership dues
- The goal is that 50% or more of chapter members will be members of AONN+
- Nurse, Social Worker, Patient Navigator, or other professionals responsible for navigating patients through the oncologic continuum of care are welcome to participate
- We encourage our members to participate openly and engage in discussion during our meetings
- Ideally our members will attend at least 75% of our quarterly meetings
